Affordable Care Enrollment Assistance at the Malverne Library

 KEY DATES YOU SHOULD KNOW:

- November 15, 2014.  Open Enrollment begins.
Apply for, keep, or change your coverage.

- December 15, 2014.  Enroll by the 15th if you want
new coverage that begins on January 1, 2015.  If your
plan is changing or you want to change plans, enroll
by the 15th to avoid a lapse in coverage.

- December 31, 2014.  Coverage ends for 2014 plans.
Coverage for 2015 plans can start as soon as January 1st.

- February 15, 2015.  This is the last day you can apply for
2015 coverage before the end of Open Enrollment.

The Malverne Library has been designated an Enrollment Center.
This means we host trained Navigators to help you sign up for
health care through the New York State Health Exchange Network.
Here you can enroll for the first time, change your coverage
if your circumstances change,  apply for Medicaid
or Child Health Plus.
Sessions take place on the third Monday of each month.  (However the January 19, 2015 and February 16, 2015 sessions will not be offered due to holiday closures.)
No registration;  sessions will be assigned to walk-ins on a first-come,
first-served basis starting at 1:00 p.m. until 6:00 p.m.

  • NY State of Health customer service: 855-355-5777. Hours: Monday- Friday 8 a.m. to 5 p.m. As of 10/1, the hours will be: Monday-Friday 8 a.m. to 8 p.m. and Saturday 9 a.m. to 1 p.m. Or got the website: nystateofhealth.ny.gov.
    • The state has designated three lead organizations to help Long Islanders navigate the exchange. For information or to find out when and where you can meet with someone who will help you enroll, contact:
